A friend reached out a few weeks ago. She’s someone who reads everything, questions everything, and has been managing a serious chronic condition for years with more discipline than most people I know. Her husband has real health risks. They don’t make decisions lightly.

On a personal note, I’ve struggled with weight my whole life. The drug version is a synthetic analog — not bioidentical — meaning it’s an engineered copy of your natural hormone, deliberately modified to resist breakdown and last days to a week instead of minutes. Your body was never designed to run this signal that long. That’s not a flaw in the drug. It’s the whole point. But it also means the body is experiencing a sustained hormonal signal it was never designed to maintain.
2. Muscle loss is the hidden risk nobody’s talking about
These medications reduce appetite significantly. But a body losing weight rapidly — even with attentive nutritional and movement support — loses muscle along with fat. Muscle is your metabolic engine. Less muscle means a slower metabolism, which sets up the next problem.
3. Many people regain the weight — and sometimes more
When the medication stops, many people regain nearly all the weight they lost. In some cases they gain beyond their starting weight — with a higher fat-to-muscle ratio and a raised “set point.” The body has adapted. This is not a willpower failure.
